Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health's Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to end-of-life issues.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Help patients and families cope with medical diagnoses, aging-related changes, and end-of-life issues.
- Assist with discharge planning and offer counseling support.
Requirements
- Bachelor's degree with 1-3 years of experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong knowledge in social work theory and practice.
