SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide vital clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment difficulties, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and their families.
• Address common challenges including adjustment to institutional living, financial issues, family relationships, and end-of-life counseling.
• Provide support in understanding medical diagnoses, discharge planning, and coping with aging-related changes.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong knowledge of theory and practice within the social work field.
