Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a PRN Social Worker at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling on end-of-life matters.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families in coping with medical diagnoses, discharge planning, and aging-related changes.
• Collaborate with interdisciplinary teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
