SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of emotional and practical challenges. As a valued member of our Allied Health team, you will play a vital role in supporting individuals during difficult times and helping them navigate complex healthcare decisions.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address various issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
• Collaborate with interdisciplinary teams to develop comprehensive care plans.
• Assist with discharge planning and support patients in transitioning to the next level of care.

Requirements

• Bachelor's degree in Social Work or a related field.
• 1-3 years of experience in clinical social work.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal skills and empathy towards patients and families.
