Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges. Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Assist individuals in coping with adjustment to institutional living, financial concerns, family dynamics, and medical conditions.
• Provide counseling and support related to aging, discharge planning, and end-of-life issues.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• Must possess LBSW, LMSW, or LGPC certification.
• Demonstrated knowledge of social work theory and practice.
• Strong communication and interpersonal skills.
