Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges such as institutional living adjustments, financial concerns, family relationships, and counseling on end-of-life issues.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address a range of issues including financial concerns, family dynamics, aging-related changes, and end-of-life planning.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Sound knowledge of clinical social work theory and practice.
