Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. This position involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to various medical and emotional issues.

Responsibilities

• Conduct clinical social work services and psychosocial assessments.
• Address common problems related to adjustment, finances, family dynamics, and counseling needs.
• Participate in discharge planning and support patients and families through difficult situations.

Requirements

• Bachelor's degree with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Working knowledge of social work theory and practice.
