SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide critical clinical social work services to patients and their families, addressing various psychosocial needs and challenges. This role involves assisting individuals with adjustments to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Provide psychosocial assessments and counseling to patients/residents and families.
• Address common issues such as adjustment difficulties, financial concerns, and family relationships.
• Assist individuals in understanding and accepting medical diagnoses and procedures.
• Develop discharge plans and provide support related to death and dying.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
