Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, covering a wide range of common issues.

Responsibilities

• Offer clinical social work services to address various challenges faced by patients and families.
• Conduct psychosocial assessments to provide appropriate support and counseling.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification preferred.
• Familiarity with social work theory and practice.
• Excellent interpersonal and communication skills.
