SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, financial concerns, family relationships, medical understanding, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist patients in adjusting to institutional living and addressing financial concerns.
• Support individuals in understanding medical procedures and treatments.
• Develop discharge plans and offer counseling on end-of-life issues.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
