SOCIAL WORKER PRN

Job Description

We are currently looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, Maryland. This position is PRN (As Needed) and includes providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Perform psychosocial assessments to identify patient needs.
• Provide counseling and support for patients and families facing challenging situations.
• Assist with discharge planning and transition of care.
• Collaborate with the healthcare team to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
