Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, and counseling on end-of-life issues.

Responsibilities

• Offer clinical social work services to patients and families.
• Assess and address common psycho-social issues.
• Provide counseling and support for end-of-life matters.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
