SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. As part of our Allied Health department at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families.

Responsibilities

• Assess and address common psychosocial issues faced by patients and families.
• Provide counseling on adjustment to institutional living, discharge planning, and end-of-life care.
• Collaborate with healthcare teams to support patients' holistic well-being.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
