SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. As a member of our Allied Health department at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families.

Responsibilities

• Offer support and counseling to individuals and families facing various challenges such as adjustment to institutional living, financial concerns, and end-of-life issues.
• Conduct psychosocial assessments to address emotional, social, and practical needs of patients.
• Assist in discharge planning and facilitate referrals to community resources as needed.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience as a social worker.
• Licensed Social Worker (LBSW, LMSW, LGPC).
• Strong communication and interpersonal skills.
