SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join the LifeBridge Health team in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, focusing on various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning. The ideal candidate will have a Bachelor's degree, 1-3 years of experience, and relevant certifications such as LBSW, LMSW, or LGPC.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Offer counseling and support regarding medical diagnoses, aging, and end-of-life issues.
• Develop discharge plans and coordinate with healthcare teams and community resources.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
• Strong communication and organizational skills.
