Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of common issues, including adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and end-of-life care.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
Requirements
- Bachelor's degree and at least 1-3 years of experience in social work.
- LBSW, LMSW, or LGPC certification.
- Strong knowledge of social work theory and practice.
