Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As part of our team, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common problems with compassion and professionalism.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address patient and family needs.
• Assist with discharge planning and support patients in adjusting to medical procedures and treatments.
• Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work or a related field.
• 1-3 years of relevant experience.
• Licensed (LBSW/LMSW/LGPC) social worker.
