Job Description
We are looking for a Social Worker PRN to join our team at LifeBridge Health, specifically at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical services and conduct psychosocial assessments for patients/residents and their families. Your role will involve addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on death and dying.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments and address common challenges.
- Collaborate with healthcare teams to ensure the well-being of patients and residents.
Requirements
- Bachelor's degree and 1-3 years of experience in social work.
- LBSW/LMSW/LGPC certification preferred.
- Working knowledge of social work theory and practice.
