Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position is PRN (as needed) and involves providing clinical social work services and psychosocial assessments to patients, residents, and their families. Your role will include addressing various challenges such as adjusting to institutional living, financial concerns, family relationships, health-related issues, aging, discharge planning, and end-of-life counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
