Job Description
We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist patients and families in coping with medical diagnoses, aging, and end-of-life issues.
- Develop and implement discharge planning.
Requirements
- Bachelor's degree in social work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
