Job Description

We are looking for a compassionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services, conducting psychosocial assessments for patients and their families. Your responsibilities will include addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments for patients/residents and their families.
• Provide counseling and support related to various challenges faced by individuals and families.
• Assist in discharge planning and end-of-life care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
