SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our dedicated team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Assist patients with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to address patients' social and emotional needs.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW/LMSW/LGPC.
