Job Description

We are looking for a passionate Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services, conduct psychosocial assessments, and offer counseling to patients and families facing various challenges.

Responsibilities

• Deliver psychosocial assessments to address common patient and family issues.
• Provide counseling on adjustment, financial concerns, family relationships, and end-of-life matters.
• Assist in discharge planning and support patients with transitioning.
• Collaborate with healthcare professionals to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Sound knowledge of social work theory and practice.
