SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges they may encounter. Your responsibilities will include assisting with adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Assist with adjustment to institutional living, financial concerns, family relationships, and discharge planning.
• Offer counseling related to difficult medical diagnoses and procedures as well as end-of-life care.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong communication and interpersonal skills.
• Empathy and ability to address sensitive issues with patients and families.
