SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As part of our team, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges related to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life issues.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and offer support during difficult life transitions.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.
• Maintain accurate records and documentation of patient interactions.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in clinical social work.
• Licensed Social Worker (LBSW, LMSW, LGPC).
• Strong communication and interpersonal skills.
