SOCIAL WORKER PRN

Job Description

We are seeking a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, helping them navigate challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life issues.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and offer support during difficult times.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of social work experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
