Social Worker PRN

We are currently looking for a dedicated Social Worker to join our team at LifeBridge Health. This position is based in Baltimore, MD at Levindale Hospital with a focus on clinical social work services and psychosocial assessments for patients and their families.

Responsibilities

• Provide support for patients/residents and families regarding various common challenges.
• Address issues related to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Requirements

• Bachelor's degree or equivalent with 1-3 years of experience.
• LBSW, LMSW, or LGPC certification.
