Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. This position provides essential clinical social work services, conducting psychosocial assessments, and offering support to patients, residents, and their families. The role involves addressing various common challenges such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common challenges faced by patients and families.
• Offer counseling and support related to medical conditions, aging, and end-of-life care.
• Work collaboratively with healthcare teams to facilitate discharge planning.

Requirements

• Bachelor's degree with LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in social work.
• Strong understanding of clinical social work practices.
• Ability to provide compassionate care and support to patients and families.
