Job Description

We are looking for a passionate SOCIAL WORKER PRN to join our team at LifeBridge Health. This position, based in Baltimore, MD at Levindale Hospital, involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common adjustment challenges related to medical diagnoses, aging, discharge planning, and end-of-life counseling.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
