Job Description
We are looking to hire a PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities will include addressing a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical diagnoses and discharge planning.
Requirements
- Bachelor's degree or equivalent professional knowledge
- 1-3 years of relevant experience
- LBSW/LMSW/LGPC certification
