Your role will involve addressing various challenges such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support for issues related to medical conditions, aging, and end-of-life care.
• Collaborate with healthcare teams to develop discharge plans.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
