Job Description

We are looking for a passionate Social Worker PRN to join LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, helping them navigate various challenges and transitions in a hospital setting.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Assist with discharge planning and end-of-life counseling.
• Address issues such as adjustment to medical diagnoses, financial concerns, and family dynamics.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed as LBSW/LMSW/LGPC.
• Strong communication and interpersonal skills.
