SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join LifeBridge Health in Baltimore, MD. As a key member of the healthcare team, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues related to aging, medical diagnoses, and end-of-life care.
• Collaborate with healthcare professionals to develop comprehensive care plans.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of social work experience in a healthcare setting.
• Strong knowledge of social work theory and practice.
