SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities will include addressing common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling on medical issues.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Offer counseling on a variety of challenges patients and families may face.
• Assist with discharge planning and support related to death and dying.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
