SOCIAL WORKER PRN

Job Description

We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems faced by patients and families.
• Offer counseling and support on a variety of issues related to health and well-being.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience in the field.
• LBSW, LMSW, or LGPC certification.
