SOCIAL WORKER PRN

Job Description

We are currently looking for a compassionate SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. As a member of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common issues including adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
• Collaborate with healthcare professionals to support patients and families in difficult situations.
• Execute discharge planning and other social work interventions effectively.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in the field.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
