Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, assisting them with various common challenges. Responsibilities

• Provide psychosocial assessments and clinical social work services to patients and their families.
• Address adjustment to institutional living, financial concerns, family relationships, and other related issues.
• Assist patients in understanding medical diagnoses, procedures, and treatment options.
• Engage in discharge planning and provide counseling on end-of-life care and bereavement.

Requirements

• Hold a Bachelor's degree and possess basic professional knowledge in clinical social work.
• Have 1-3 years of relevant experience in a healthcare setting.
• Be a licensed LBSW, LMSW, or LGPC.
