Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position involves providing essential clinical social work services, including psychosocial assessments, to patients, residents, and their families. The role focuses on addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling on medical issues, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of professional experience.
• LBSW, LMSW, or LGPC certification.
