Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, helping them navigate various challenges and transitions in their lives.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Assist with discharge planning and end-of-life care.
• Address issues related to adjustment, financial concerns, family relationships, and more.
• Collaborate with healthcare professionals to ensure holistic patient care.

Requirements

• Bachelor's degree and relevant social work licensure (LBSW/LMSW/LGPC).
• 1-3 years of experience in a healthcare setting.
• Knowledge of social work theory and practice.
• Strong communication and interpersonal skills.
