SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN (as needed) Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a range of common issues such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments to address various issues.
• Offer counseling and support related to aging, family relationships, and end-of-life care.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
